Home and Community Based Services

Our home and community based therapy services are designed to promote independent management of conditions or challenges which interfere with enjoyment of everyday life or fulfillment of daily responsibilities.

Home and community based services allow our therapists to evaluate how our clients function within their natural environment. This allows our therapists and clients to develop meaningful goals and practice skills in real time, so that we can anticipate challenges or barriers and troubleshoot solutions together. Treatment plans will evaluate and address:

  • Safety awareness in home

  • Home organization  

  • Laundry and light meal prep

  • Medication management 

  • Housekeeping and home maintenance 

  • Budgeting 

  • Home modifications

  • Low vision solutions 

  • Need for Durable Medical Equipment

Our educational support and career consultation services provide insights into how our client's background, independent living skills, physical strength/mobility, cognition and more impact educational and career planning. We support our clients by identifying how barriers to independence at school or in  the workplace may limit potential. We work with you to develop a plan for how to advocate for the right supports and accommodations that foster accessibility and independence at school or in our client's workplace. We also work with educational and employment teams to provide integral knowledge to drive IEP, 504, and employment success goals.

Our services address finding solutions to barriers that impact success as we work do develop a supportive plan to address:

Mighty Oak's Occupational Therapy services for children and adolescents focus on a building a strong foundation for independent management of life's day to day changes and challenges. We work with children and adolescents in both individual and group settings for greater consumer access to therapeutic intervention.
